Standard Scrip Order Form
This Form has the all-time favorites that we sell the most of in our scrip program.

Specialty Scrip Order Form
This Form has the Specialty scrip that is convenient to use for gifts
or just to have around for special occasions.

Please return the completed Order Form to the OLPH Rectory or the School Office.
It can also be faxed in at (619) 443-1733, but make sure it is properly completed!
